This very active 39-year-old female presented with a history of morbid obesity.  She previously underwent gastric bypass surgery and lost approximately 130 pounds.  Her weight stabilized at 135 pounds, which corresponded to an approximate BMI of 25.  She presented with concerns about her abdomen,  breasts, and thighs.

Examination reveals significant laxity of the abdominal area extending into the flanks and buttock area as well.  Lipodystrophy of the thighs was noted with associated skin excess.  Her breasts demonstrated loss of volume with development of significant breast ptosis.  A staged operative plan was developed to address each of these concerns.

The patient initially underwent lower body lift surgery combined with mastopexy surgery.  Her surgery was complicated by postoperative bleeding edge required a return to the operating room for control of the hemorrhage.  She subsequently did well with rapid healing.

A second surgery is planned at which time.  She will undergo circumferential thigh liposuction and bilateral breast augmentation.  The liposuction procedure will be performed in order to reduce the volume and circumference of her thighs in preparation for a later medial thigh lift procedure.
